Registration Procedures

To register for the 2019 Diabetes Educational Symposium, please fill out the registration form below.

Registration fees may be paid by credit card via PayPal, or by check.

If paying by check, the check must be made out to the Diabetes Coalition of Palm Beach County. Please indicate on the check memo line, Diabetes Symposium Registration Fee. The check should be mailed to:
